During the initial weeks of the school year or when a new student enrolls, crucial information is shared. Various forms are sent to caregivers via mail and email, requiring completion. To help you organize, several of the essential forms are located below.
- Digital Resources Consent: Some FCPS-approved digital resources require parent/caregiver consent before they can be used by students. Find the list of resources that require consent on the Digital Resources - Parent Consent page. This page also includes directions on how to provide permission so your child(ren) can access all resources provided by their teachers.
- Student Rights and Responsibilities (SR&R): This document explains expectations for student behavior and adult responses. Parents using SIS ParentVUE will be prompted to sign the document when they login to that platform. A printable version of the document with a signature sheet is available on the .
- Opt-Out: The Opt-Out booklet is online or available from your child's school. It contains information on:
- Parents' rights to inspect and consent or opt-out before their child participates in certain surveys.
- Parents' rights to inspect instructional materials.
- Rules regarding student records and disclosure of student information (including photo/video of students) and the right to opt-out.
- Parents' rights to opt out of the Social-Emotional Learning Screener, physical exams not required by state law, career or social-emotional counseling, Library Equity Access Pass Project, and/or Tutor.com.
- The process by which complaints may be filed if parents or students believe their rights have been violated.

Required Forms
There are many tasks and forms for parents/guardians to complete at the beginning of the school year or when registering or transferring from another FCPS school during the year. We have organized the forms into those required and those that are optional.
Online Verification Update Packet
Online Verification Update Packet (OVU)can be accessed from ParentVUE accounts. OVU allows parents to conveniently update phone numbers, emergency contacts, health conditions, and immunization dates for students. It is accessible through ParentVUE accounts.
Returning families or whenever there is a change with existing accounts, please log in to review and update your information before the start of the school year so that we have your family's most up-to-date information.
Emergency Care Card: Updated Annually
Review for accuracy and update if changes are needed. ParentVUE account required.

Health Information Form: Only for Students with Health Concerns
While you are updating your Emergency Care Card Information make sure to fill out the “Current Health Conditions” and/or “Current Medical Care Details" in addition to submitting this form.

Parent/Guardian Digital Consent
Teachers will use a variety of digital resources to help students learn during the 2023-24 school year. These digital resources have been checked and approved by Fairfax County Public Schools. Please provide your consent for your student to use these resources through the Parent Digital Consent System.
Explanation of Form:As part of our commitment to protect student data, FCPS is required to secure parental permission for student use of software/applications for which FCPS does not have a confidentiality agreement. For ease of access, an online process has been developed to collect this information digitally and provide school-based reporting.
You can find more information about FCPS’ commitment to student privacy atDigital Privacy in FCPSand a list of all approved digital tools is viewable online in theFCPS Digital Ecosystem Librarywhich has links to the vendors’ privacy notices and other useful information.
This permission will expire at the end of this school year and can be revoked at any time by contacting the school. If you have questions about any of the digital resources listed below, please contact your child’s school.
The SR&R handbook is located online in SIS. Parents and guardians are responsible for acknowledging they have received and reviewed the SR&R and should be prompted to do so when they log into their SIS Parent Account. Students will be reviewing the SR&R during the month of September.
Parents/guardians will digitally sign the SR&R inSIS: ParentVUE. Every parent/guardian with a ParentVUE account, regardless of relationship to the student, will see the SR&R upon login to PVUE until the SR&R acknowledgment has been completed. If a parent chooses, he/she/they may “skip” the SR&R acknowledgment by clicking on “Remind me later!”.The next time the parent/guardian logs in the SR&R acknowledgment will appear again.
The parent/guardian will NOT be required to click on the pdf document link to download/view the SR&R packet before the parent/guardian will be able to digitally sign and acknowledge the SR&R. The name in the digital signature box must match the parent first and last name in SIS exactly before the signature will be accepted. Confirm the exact match with the "Welcome Message" in the top right of the screen where the parent's name is printed on the screen.
CLICK on "SEND FORM." Changes will be updated electronically in our Student Information System (SIS). You do not need to return the paper copy.
Use ofSIS: ParentVUEto submit the SR&R signature sheet is preferred. If you need assistance accessing the SR&R Resources or the Signature Sheet please contact the school for guidance.

Family Life Education (FLE) Opt-Out Forms
Forestville Elementary provides a comprehensive, sequential Family Life Education (FLE) program for students in grades K-6. Instruction is seen as a partnership among parents and guardians, the school, and the community in supporting the learning essential to the development of strong families, positive relationships, and a healthy community.
Parents/guardians may choose to opt their child out of all or part of the Family Life Education program. Students who are opted-out are provided with age-appropriate, nonpunitive alternative health instruction, and every effort is made to foster respect for family choices. If you wish to opt your child out of all or part of FLE for this school year, opt-out forms are available online.
If any apply, you may complete and email forms to Amani Kirk: @email. If you are unable to submit the forms digitally they can be dropped off at the front office.

🔍 Lightspeed Parent Report on Child’s Device Activity
FCPS has tools to help parents know more about their child’s activity on any FCPS device. These tools are provided at no cost to families by Lightspeed, the school division’s internet content filtering service.
Parents may sign up for aWeekly Parent Report, which will provide a list of the top sites your child visited. The report also offers parents the ability to sign up for free access to the Internet Use Parent Portal.Through the portal, parents may pause their child’s access to the internet on their FCPS device during non-school hours.

Military-Connected Student Form

My School Bucks
My School Bucks is an online payment service that provides a quick and easy way to pay for school-related purchases. You will be able to pay for a variety of school fees, including field trips, meals, and using credit/debit cards or electronic checks.
Optional Medical Forms (To Be Completed Annually)
Epinephrine Authorization
TheEpinephrine (EpiPen) Authorization Formis specifically for students who have a prescribed epi-pen. All medications are required to have the appropriate authorization form.
Inhaler Authorization
TheInhaler Authorization Formis specifically for students who have a prescribed inhaler. All medications are required to have the appropriate authorization form.
Medication Authorization
As needed, over-the-counter medications may be transported to school (directly to the health room). The medication must be in the original unopened container and accompanied by an appropriate Medication Authorization Form.
All prescription medications must be transported to school by a parent or designated adult.

Any parent/guardian associated with a student with an SIS ParentVUE account can update these opt-outs as needed by going to the "Student Info" tab and clicking "Edit Information."
SIS Parent Account
TheSIS Parent Account(otherwise known as “ParentVUE”)is a secure portal for parents to view information about student schedules, demographics, attendance, current class performance, and grade reports.
Parents can now also update student information (phone numbers, emergency contacts, health conditions, and immunization dates) online using the Online Verification Update Packet (OVU) in their ParentVUE account. Once you log in, look for the "Online Packets" tab in the upper right of the screen, then select the "Online Verification/Update" form in the drop-down menu. Click "Begin Packet".
Before school begins parents should log in to their account to review and update all necessary information.

News You Choose is a subscription-based, email communication tool used to share communications such as newsletters, events, news updates and other topics. Teachers use Schoology to post their classroom materials online, to provide a secure forum for students to discuss their ideas and collaborate on projects, and to assign and collect homework electronically. It helps students stay organized and it keeps the class connected.

A Schoology parent account gives you access to:
